Home care also referred to as; senior care and in-home caregiving – is a service provided by Amara Health Care that regularly sends a professional Caregiver to a person’s home or care community to help them accomplish activities of daily living, such as bathing, doing the laundry, meal preparation and more.

  • Provides individualized care plans with one-on-one attention.
  • Helps clients maintain their independence and dignity as they age.
  • Offers the vital companionship needed to prevent loneliness and isolation.
  • Helps clients age in place.

When you choose home care, you’ll receive an individual care plan designed to help the entire family achieve quality of life goals. The plan will be tailored to individualized needs – based on your input. Once a schedule is in place, a caregiver will arrive at the appointed time each day or week to provide compassionate, personalized care.

What are Self-Directed Services (SDS)?

SDS is a service delivery model through Maryland’s Developmental Disabilities Administration (DDA) that allows individuals with disabilities more control over their services. Participants can hire, train, and manage their own support staff.

  • Individuals enrolled in one of Maryland’s DDA Medicaid Waivers (Community Pathways, Community Supports, or Family Supports).
  • Must be approved to self-direct by the DDA.
  • Must have a Person-Centered Plan (PCP) that supports self-direction.

  • Community Development Services (CDS)
  • Personal Supports
  • Employment Services
  • Respite Care
  • Support Broker Services
  • Transportation 

Yes. You can select your own support staff, including family members (with restrictions), and work with a Fiscal Management Service (FMS) to process timesheets and payroll.

A Support Broker helps individuals manage their SDS services. They assist with hiring, training, scheduling, and other administrative duties, but the participant or representative retains control over decisions.

  • Let your CCS know you’d like Amara Health Care as your provider.
  • Your CCS or Support Broker will send us a referral.
  • We’ll schedule an intake and match you with a qualified DSP based on your needs.

Yes. Our Direct Support Professionals are:

  • CPR/First Aid certified
  • Trained in person-centered practices
  • Screened and background checked
  • Provided with ongoing education to support diverse needs

Yes! SDS allows for flexibility. Services can be delivered:

  • In your home
  • In the community (library, job site, gym, etc.)
  • Wherever your goals can be supported
  • Yes, we support youth and adults with developmental disabilities, depending on the DDA waiver they are enrolled in. 

Yes. You have the right to choose or change your provider at any time. Your CCS can help initiate the transition to Amara Health Care.

What are the eligibility requirements for a veteran to get home care under the VACCN?

To receive home care under the VACCN, the Veteran must first be enrolled in the VA Health System and usually demonstrate a need for assistance with one or more ADLs (Activities of Daily Living).

Veteran home care refers to non-medical or medical services provided in a veteran’s home to support daily living, improve quality of life, and promote independence.

Veteran home care may include:

  • Personal care (bathing, dressing, grooming)
  • Meal preparation
  • Medication reminders
  • Light housekeeping
  • Mobility and transfer assistance

Yes. The VA may cover home care services through programs like:

  • Veteran Directed Care (VDC)
  • Homemaker and Home Health Aide Program
  • Community Care Network (CCN)
  • Aid & Attendance Pension Benefit

Yes, through the VDC program and certain VA caregiver support programs, family members can be paid for providing care.

Yes. Amara Health Care is experienced in serving veterans and works closely with VA case managers, the Community Care Network, and Veteran Directed Care programs.

We provide veteran home care services across Harford, Cecil, Kent, and Baltimore Counties.

Once a referral is made and eligibility is confirmed by the VA, services can typically begin within 7–14 days depending on care needs and program availability.
